What is an advance directive?+

An advance directive is a written statement of your wishes about medical treatment if you cannot speak for yourself. It usually includes a living will and a healthcare power of attorney.

Who should I name as my healthcare agent?+

Someone who knows your values, will follow your wishes even under pressure, can stay calm in a hospital setting, and is reachable in an emergency. It does not have to be a spouse or oldest child.

Where should copies be kept?+

Give copies to your healthcare agent, primary doctor, and hospital. Keep one on your refrigerator for paramedics. A copy in a safe-deposit box alone is not enough — no one can reach it in a crisis.
